Register with us as a New Patient

How to Register

We actively welcome new patients to our Practice. You can register at reception by using a registration form and supplying us with proof of identity and address. All new patients will also be asked to complete a lifestyle questionnaire and are advised to book an appointment with the practice nurse or HCA for a health check within 28 days of registering.

The health check gives us the opportunity to review how we can best meet your health needs and also ensures you will have access to our full range of services.Failure to book and attend the Health check screening may mean that you cannot access a GP.

Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.

There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

Please note that if you were previously registered with a Doctor outside the UK then the Practice will not have access to your medical notes.  All patients who register from abroad should contact their previous Doctor to obtain copies of their notes.

Allocated Named GP

You may be aware that all practices are required to provide their patients with a named GP who will have overall responsibility for the care and support that our surgery provides to them.

The allocated named GP will have overall responsibility for the care and support that our surgery provides to you. This does not prevent you from seeing any GP in the practice as you currently do.

Patients have been allocated their named GP in the following way*:

Patients with surnames beginning with the letter:

  •  A to G will have Dr Ramachandra as their named GP

Patients with surnames beginning with the letter: 

  •  H to Q will have Dr Gupta as their named GP

Patients with surnames beginning with the letter:

  • R to Z will have Dr Hu as their named GP

Register online

We are using an NHS online service called Register with a GP surgery that makes it easy to register with this GP surgery. This secure system will email us your registration form.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It reduces our administrative workload and makes it easier for you to register.

The National Care Record Service (NCRS)

The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.

Organ Donation
